Tadalafil Longevity Gummy Guide
The first-ever Tadalafil gummy built for longevity is also simple to use: a calm, step-by-step walkthrough for storing and taking your daily gummy at home, so the routine is settled from the very first dose.
Before you begin
Starting a daily gummy for long-term vascular support can come with real questions. Is once a day really enough? How do you know it is working when there is nothing dramatic to notice? This guide is here so you don’t have to sort your questions out alone.
Tadalafil + Vitamin D3 Longevity Gummy pairs a low daily dose of Tadalafil with 600 IU of Vitamin D3 in a once-a-day habit, for men and women who want daily support for healthy blood flow as they age. It does not replace guidance from your provider, and benefits and effects vary from person to person. This is a room-temperature product; storage details are below. It is offered as adjunctive, supportive therapy: keep up your ongoing care with your primary care provider and any specialists for the diagnosis and management of underlying conditions (the FDA disclaimer is at the bottom of this guide).
What this may help with
It’s normal to feel a little uncertain about starting something new. This gummy is built around one pathway involved in blood flow throughout the body, which is why it is used by men and women in longevity-focused care as well as for erectile and urinary function. Here is what a daily low-dose gummy is built to do.
Responses vary from person to person, and consistency is what makes a daily protocol work.
Important safety information:
Daily low-dose Tadalafil is well tolerated by most people, but a few symptoms mean you should stop and get help right away.
Stop and seek help if you experience:
- An erection that will not go away, or that lasts longer than 4 hours (priapism). Left untreated, this can cause lasting damage to the penis.
- Sudden loss of vision in one or both eyes.
- A sudden decrease or loss of hearing, sometimes with ringing in the ears or dizziness.
- Chest pain, or worsening shortness of breath, during or after sexual activity.
- Dizziness, lightheadedness or fainting, which can mean your blood pressure has dropped too far.
- Swelling of the face, lips, tongue or throat, hives, or difficulty breathing (signs of an allergic reaction).
If symptoms are severe, call emergency services (911). For milder but persistent concerns, contact your care team before your next dose.
This is normal
A mild headache, warmth or flushing, a stuffy nose, heartburn, or back or muscle aches is commonly reported with PDE5 inhibitors, and usually eases as the body adjusts. See Side effects below for the complete list and typical timing.
About your shipment
It’s normal for your order to arrive at room temperature
This is not a refrigerated product: it ships without ice packs and never goes in the fridge or freezer. Orders typically arrive within 5 to 7 business days of approval, as a black jar with a white pharmacy label, compounded by Precision Fulfillment Pharmacy, a state-licensed compounding pharmacy.
Each gummy contains 2.5 mg Tadalafil and 600 IU vitamin D3. If you have questions regarding ingredients or dosing, be sure to reach out to your care team. Your own strength and instructions are printed on the label and live in your AgelessRx portal.
Storing your medication between doses
Keep the gummies in the original jar with the lid tightly closed, at room temperature. Keep them away from heat and humidity, so avoid the bathroom, a sunny windowsill, or the laundry room. Because this gummy looks like a sweet, a closed cupboard is a better home for it than a counter or bedside table. Keep out of reach of children and pets.
Check the beyond-use date on your label
Compounded medications carry a beyond-use date, a date which indicates when a compounded medication can no longer be safely used. For this gummy it is 180 days from the date it was compounded. Discard anything left after that date even if the jar looks fine, and always go by the date printed on your own label. If your order arrives damaged, leaking, or melted, see Troubleshooting below for what to do.
Dosing
Take one gummy once daily, or as directed by your provider, at about the same time each day. What matters most is consistent daily use. Standard dosing is 2.5 mg, one gummy a day, unless your provider has told you otherwise.
Don’t adjust your dose on your own
Your provider can help decide if an increased dose may be right for you. Skipping a day and doubling up before an evening you have planned is never recommended or beneficial: it raises the odds of headache, flushing and low blood pressure, and it breaks the steady level a daily protocol is built on.
Missed a dose?
Take the next day as scheduled. Do not double up.
Why 2.5 mg and not 5 mg?
Starting low is deliberate: it keeps side effects like headache and flushing mild while the body adjusts. Your provider may increase your dosage to 5 mg if 2.5 mg is not providing the expected benefits after a fair trial, or if they determine an increase is clinically appropriate. If a medication you take raises Tadalafil levels (see Safety and interactions below), your provider will keep you at 2.5 mg. Message your care team to discuss possible dosing options, and never change the dose yourself.
What your supply covers
Your prescription is written to align with your subscription type for either a monthly or quarterly supply. Always go by the prescription instructions printed on your own label. Your approval lasts 18 months, and a check-in is what lets your provider re-issue the next prescription, which is why check-ins are not optional.
The most common reason people pause this treatment is having too much on hand: the supply and the routine have drifted apart, either doses are being missed or a shipment landed early. Message your care team to delay, pause or reassess your supply before it passes its beyond-use date.
Ramp-up timeline
Taking your gummy
There is nothing to measure, mix or prepare. A few simple habits are all it takes.
One thing this does not do
Tadalafil supports blood flow during arousal. It does not create an erection on its own and it does not increase desire, so sexual stimulation is still needed for it to work. Expecting an effect without that is the most common reason a daily protocol looks like it is failing when it is not.
Everyday habits & disposal
A daily oral treatment asks very little of you, but these habits are what keep it working and keep everyone else safe around it.
- Log your doses. A quick note of the date and how you felt makes dose decisions easier and helps spot a missed-dose pattern.
- Complete your check-ins. Periodic check-ins are how your provider confirms the dose is still right and keeps refills moving. If a check-in link will not open, message your care team.
- Dispose of what you do not use. Do not flush gummies and do not put them loose in the trash where a child or a pet could find them. Mix them with something unappealing such as used coffee grounds in a sealed bag, or use a pharmacy take-back program.
- Never share it. The dose prescribed is for you and you alone.

Signals to adjust your dose
Let your response set the pace, and let your provider make the change. These are the cues worth sharing. When in doubt, message your care team.
Message your care team ›Signs it may be worth asking about a change
- You have taken it daily for two to four weeks and have not noticed the difference you hoped for.
- You are tolerating it well, with no headache or flushing after several weeks.
- Your provider already raised a possible step up at a check-in.
Signs to flag before your next dose
- Headache or flushing that is not easing after the first week or two. Let us know.
- Dizziness or lightheadedness. Do not wait to notify your care team. This is on the emergency side effect list at the top of this guide, and is more likely if you also take an alpha-blocker or another blood pressure medication.
- Back pain or muscle aches that persist beyond a couple of days.
- Anything on the safety information list at the top of this guide. Side effects under the list to stop and seek help for are not dose conversations, they are side effects that should be immediately evaluated.
Tips for staying comfortable
Hydrate, and give it a week
Early minor side effects often ease as the body adjusts.
Stand up slowly
Tadalafil can lower blood pressure a little, so rise slowly. Lightheadedness or fainting is the emergency-list item above.
Safety and interactions
Do not use this gummy if any apply
- You take nitrates in any form, prescribed or otherwise, such as nitroglycerin tablets, sprays or patches, or isosorbide.
- You take a guanylate cyclase stimulator such as riociguat, or another PDE5 inhibitor such as sildenafil, vardenafil or any other Tadalafil product.
- You take cisapride.
- You have used recreational drugs in the last three months, in particular cocaine or inhaled nitrites (sold as “poppers” or “rushers”). Combined with Tadalafil these can cause a profound drop in blood pressure.
- You are under 18, pregnant, or breastfeeding.
- You have active cancer.
- You have a cardiovascular condition: angina, heart failure of class III or greater, cardiomyopathy, an irregular heart rhythm, a previous heart attack, a heart or artery malformation, or a bleeding disorder.
- A clinician has told you that sexual activity is not advisable for you because of your heart.
- You have a blood cell condition: a bleeding problem, sickle cell anemia, multiple myeloma or leukemia.
- You have had a stroke.
- You have Peyronie’s disease, or a history of an erection lasting longer than 4 hours.
- You have severe vision loss, NAION, or retinitis pigmentosa.
- You have a history of spinal cord injury, or previous prostate or pelvic surgery. Those are managed by a urologist.
- You have active tuberculosis, primary hyperparathyroidism or sarcoidosis. These relate to the vitamin D3 in the formulation.
- You have a serious hypersensitivity to Tadalafil or to any component of the formulation.
Talk to your provider first if any apply
- You take an alpha-blocker such as tamsulosin or doxazosin, or any other blood pressure medication. The blood-pressure lowering can be additive.
- You take a medication that raises Tadalafil levels, such as ketoconazole, ritonavir, fluconazole or erythromycin. Your provider will keep you at 2.5 mg.
- You take a medication that speeds Tadalafil up, such as rifampin, which may reduce how well it works.
- You have reduced kidney function, or liver enzymes more than three times the normal range.
- Your blood pressure runs low, below 90/60, or high, at or above 160/100.
- You already take a vitamin D supplement, a multivitamin containing vitamin D, or cod liver oil, so the daily total is accounted for.
Drug interactions
The nitrate interaction is the serious one: combined with a nitrate, a guanylate cyclase stimulator such as riociguat, another PDE5 inhibitor, or cisapride (see the contraindications above), Tadalafil can cause a sudden, severe drop in blood pressure that can lead to fainting or even death. Never combine any of those.
- Alpha-blockers (tamsulosin, doxazosin) and other blood pressure medications, may add to Tadalafil’s own small blood pressure lowering effect. Your provider will manage this.
- Grapefruit and grapefruit juice, may change how much Tadalafil is absorbed.
- Alcohol. This is not a formal contraindication, but roughly 5 units or more alongside Tadalafil significantly raises the risk of dizziness on standing, headache and a racing heart.
Tadalafil can remain in the body for up to two days after a dose, and longer with kidney or liver problems or certain medications. Tell any clinician treating you, including in an emergency, that you take Tadalafil daily. On your intake form and at every check-in, list every medication, herb, non-prescription drug and supplement you use.
Side effects
Most side effects are mild. With Tadalafil they typically appear within 12 to 24 hours of a dose and usually resolve on their own within about 48 hours, and daily low-dose use is generally associated with fewer and milder side effects than higher as-needed dosing.
Common
- Headache
- Heartburn or indigestion
- Diarrhea
- Flushing, a feeling of warmth
- Nosebleeds
- Difficulty falling or staying asleep
- Numbness, burning or tingling in the arms, hands, feet or legs
- Muscle aches or back pain
- Dry mouth
- Nasal congestion or sinus pressure
Less common
- Sensitivity to light, or blurred vision
- Changes in color vision, a blue tinge on objects or trouble telling blue from green. Avoid driving at night if this happens
- Palpitations or a mildly fast heartbeat
- Nausea or loose stools
- Fatigue
Rare, all need emergency care
- Sudden severe loss of vision (NAION)
- Sudden or rapidly worsening blurred vision
- Sudden decrease or loss of hearing
- Ringing in the ears
- Dizziness or lightheadedness
- Fainting
- Chest pain
- Worsening shortness of breath
- An erection that is painful or lasts longer than 4 hours (priapism)
- Itching or burning when you urinate
- Rash, allergic reaction or anaphylaxis
- Severe low blood pressure
- Swelling in the feet, ankles or legs
Common and less common effects are handled with simple symptom relief and usually ease as the body adjusts. If they persist past the first couple of weeks, worsen, or you are simply not sure, message your care team before your next refill. Everything in the rare column needs emergency care right away. Go to an emergency room, or call 911 if symptoms are severe, even ones that seem easy to shrug off.

About the vitamin D3 in your gummy
Tadalafil is the active ingredient doing the work in this gummy. The vitamin D3 plays a supporting role, and it helps to know what it is there for.
Why the two are together
Vitamin D3 is converted in the liver to 25-hydroxyvitamin D, and then in the kidneys to its active form, calcitriol, which behaves like a hormone (see “Vitamin D3 built in” above for what it may support).
The reason it sits alongside Tadalafil in particular is to support vascular health. Vitamin D receptors are expressed in vascular smooth muscle, in the endothelium (the thin lining of the blood vessels) and in heart muscle cells, which is the same tissue Tadalafil acts on. Vitamin D insufficiency is also common. In an observational study of 143 men with erectile dysfunction, vitamin D deficiency was present in 45.9 percent of them, and men with severe symptoms had significantly lower vitamin D levels than men with mild symptoms.3
That’s an association in one observational study, not evidence that taking vitamin D improves erectile function on its own. It’s why D3 is here as everyday nutritional support alongside the Tadalafil. If the D3 were doing the heavy lifting, this would be a vitamin, not a prescription.
At the standard dose, one gummy a day, that is 600 IU; two a day is 1,200 IU. Vitamin D is fat soluble and accumulates in the body, which is why the total across everything you take is what matters.
Before you add more vitamin D
This gummy does not replace testing or treating a diagnosed deficiency: if you want to know your level, ask about testing rather than adding more on your own. See “Talk to your provider first” above for what to tell your provider about other vitamin D you take.
