How do Tadalafil for Longevity and Tadalafil for ED differ?

Tadalafil for ED or sexual health is typically taken in a higher 10 or 20 mg dose before anticipated sexual activity, with support that may last up to 36 hours.7 Low doses of Tadalafil for Longevity use 2.5 or 5 mg taken daily to provide steadier support for the blood-flow pathway behind vascular, sexual, and urinary health. Its use for longevity is off-label and has not been reviewed or approved by the FDA.

What are the potential longevity benefits of low dose Tadalafil?

Low dose Tadalafil may help cGMP stay active longer, allowing blood vessels to relax and respond more effectively, which may support healthy circulation throughout the body.9 Large studies have found associations between PDE5 inhibitor use and fewer major cardiovascular events, lower all-cause mortality, and lower dementia risk, including in men who didn’t have diagnosed heart disease to begin with.2 9 These findings are observational, based primarily on men.

Can women take Tadalafil?

Yes. Eligible women can take low dose Tadalafil when prescribed off-label by a licensed provider. This isn’t a men’s-only mechanism: PDE5 inhibition works through the same nitric oxide–cGMP pathway in both sexes, and it’s already an approved, effective therapy for pulmonary hypertension in women.6 Randomized trials have studied 5 mg daily in women for overactive bladder symptoms, while a recent trial found improvements in sexual function and clitoral blood-flow measures in a specific female population.1 0 1 1 Evidence for female longevity specifically is still emerging, so treatment should be personalized.

Important safety information and common side effects

Generally, people who experience side effects usually get them within 12 to 24 hours after taking Tadalafil. Side effects usually resolve on their own within 48 hours. Daily Tadalafil use is usually associated with lower incidence and severity of side effects compared to as-needed Tadalafil dosing.

Most common Tadalafil side effects include headache, indigestion, back and/or muscle pain, flushing, and stuffy nose. Some more serious side effects have been reported. While rare, it is important to stop taking Tadalafil and contact a doctor right away if any of the following occur:

  • An erection will not go away (priapism). If you have an erection that lasts longer than 4 hours, get medical help right away. If not treated right away, priapism can permanently damage the penis.
  • Sudden vision loss in one or both eyes. Sudden vision loss can be a sign of a serious eye problem called non-arteritic anterior ischemic optic neuropathy (NAION).
  • Sudden hearing decrease or hearing loss. Some may also have ringing in the ears (tinnitus) or dizziness.

Possible drug interactions

Some drugs/medications may interact with Tadalafil. You should not take Tadalafil if you are taking nitrates (such as nitroglycerin tablets or patches) because the combination of these with Tadalafil could significantly lower blood pressure and lead to fainting or even death. Additionally, do not take Tadalafil if you are taking Cisapride, other ED medications (e.g. Viagra), Adcirca, or Riociguat. Also be sure to report if you smoke, drink alcohol, or use illegal drugs, as some may interact with your medication.

Other important safety information

Those who have a condition called “left ventricular outlet obstruction” from valvular problems or heart muscle enlargement may experience side effects such as fainting or light-headedness. Because those with recent heart attacks or stroke, heart pains (angina), heart failure, uncontrolled blood pressure or uncontrolled irregular heart beats, severe liver disease, and retinal eye problems have not been studied, Tadalafil (Cialis) is not recommended for these users. There may be rare occurrences of priapism or painful, prolonged erections. This is a serious condition that requires immediate medical attention. Users should seek prompt medical attention if their erection lasts longer than four hours.

After taking a single tablet, Tadalafil can remain in the body for up to two days. It can remain longer if people have problems with their kidneys or liver, or they are taking certain other medications.

Yes, mechanistically–this is the best-established part of the science. By blocking PDE5, Tadalafil may keep cGMP active longer, allowing blood vessels to relax and respond more effectively, supporting circulation throughout the body.¹³ This mechanism likely underlies the vascular, cognitive, and cardiovascular associations that are beneficial for longevity.

02 Is Tadalafil good for longevity?

The mechanism is well-established, and large observational studies have found associations between PDE5 inhibitor use and lower rates of death, cardiovascular events, and dementia–notably including men without pre-existing heart disease, which is the more novel finding.² ⁹ This evidence is observational, not from randomized trials, and can’t fully rule out that healthier men are simply more likely to use and continue this kind of treatment.

03 Does Tadalafil reduce dementia risk?

Large population studies have found PDE5 inhibitor use associated with meaningfully lower dementia rates, with one large UK cohort study finding a stronger association among men with more sustained Tadalafil use.² ⁹ This is a real and consistent signal across multiple independent studies, but it remains observational.

04 Can women take Tadalafil for vascular health?

Yes, off-label and with provider guidance. The underlying nitric oxide–cGMP mechanism is shared between sexes, and PDE5 inhibitors are already approved and effective for pulmonary hypertension in women.¹² ¹⁷ Longevity-specific evidence in women is still developing, since most large studies to date have focused on men, and our research team is leading the charge.
